PURE Dental has made every effort to be paperless. Please arrive for your first visit at least 20 minutes early to digitally complete our patient forms. These forms may include one or more of the following:
MEDICAL AND DENTAL HISTORY – includes past and previous medical history and alerts, any problems that are occurring now or recently and a list of medical RX patient is taking.

HIPPA - A signed document stating our office,will not give out any personal information of procedures completed.

 CONSENT – Signed document giving consent for doctors to evaluate patient and treat as needed.

ZOOM!® CONSENT – A signed document from patient giving consent for PURE Dental to treat patient with ZOOM!® 2 whitening.
